Data Collection Relative to the Prevention and Detection of Abuse

The primary resource used to collect data to help prevent and detect potential abuse within the WWFY are incident reports.

The WWFY recognizes the importance of using data to help prevent and detect potential abuse within the organization. The primary resource used to collect data to help prevent and detect potential abuse within the WWFY are incident reports. Incident reports are used to document injuries, workplace accidents or incidents involving 2 or more members including any suspected cases of abuse. The process for completing incident reports is as follows: 

  • Any employee can complete an incident report. The incident report must include the member details, nature of the incident, any relevant witnesses and the individual completing the incident report.  
  • Once this is completed, the supervisor must review this and then submit this to Human Resources. Human Resources will review the incident report and determine the next cause of action. For any cases involving a suspected case of abuse, Human Resources will consult with the CEO, to determine the next course of action.  
  • Incident Reports are reviewed at quarterly safety meetings to determine any potential trends, including any suspected cases of abuse and what measures the WWFY could have been taken to prevent the case.  


In addition to incident reports, WWFY will periodically conduct surveys with staff and members. These surveys will also be used as a secondary resource to help prevent and detect potential abuse within WWFY.
